Patients/Survivors

If you are a breast cancer patient/ survivor, we urge you to join us in our fight against breast cancer. Currently, we have more than 20,000 breast cancer patient/survivor participants in the HKBCR. After your successful registration, we collect your up-to-date medical profile from the first diagnosis and help you to keep track of your cancer status. Upon your request, we will compile your medical profile for your or your doctor’s perusal. Here are four simple steps on how you can participate in this first-of-its-kind territory-wide breast cancer-specific database in Hong Kong.

 

Step 1:  Download the HKBCR Consent Form.

Please read the form carefully. If you have questions please do not hesitate to call us at 3143 7353 or email hkbcr@hkbcf.org. It is important that you understand why we need your information and how it will be used.

Step 2:  Sign and return the Consent Form to the HKBCF

Please return the consent form by mail to 22/F., Jupiter Tower, 9 Jupiter Street, North Point, Hong Kong. Please do not return the completed form via facsimile. Your consent will grant us permission to access and collect your medical and treatment records from your doctor(s), and to use them in future statistical analysis.

Step 3:  Fill in the questionnaire which will be sent to you upon receipt of your consent.

We urge you to complete this questionnaire as best you can as the data collected will contribute in providing the most accurate data for future research and analyses.

Step 4:  We will contact you and/ or your physician(s) to get an annual update

Every year, we will contact you and/ or your physician(s) to get an update on your status and collect the latest medical information on your health status. We will collect the data either by email, by mail, or by phone, whichever is the most convenient for you.

Important note: We assure you that all information we collect from you, and that your medical and treatment records we obtain from your doctors and clinic, will be kept strictly confidential in accordance with the Personal Data (Privacy) Ordinance (Laws of Hong Kong Special Administrative Region, Chapter 486). The data collected will solely be used for aggregate analysis. At no time will your identity be revealed and/ or made known to a third party or be put to use for any other purposes.
